| S.Staender, MD Critical Incident Studies have long been utilized to analyze accidents and incidents in aviation for the purpose of developing countermeasures. We introduced this technique of a totally anonymous Critical Incident Reporting System, that can be used by all members of the OR team, in our hospital. In the overall more than 150 critical incidents in the system by today, elements of human error were determined in 70% of the cases. They include errors of judgment, flawed reactions due to inexperience or unfamiliar environment, failure to check equipment, inadequate preoperative assessment, inattention, haste and degradation caused by stress or illness.
