Initial dose: Bupivacaine 0.25% 8 to 12 ml
"top up"- reserve: Bupivacaine 0.25% 6 to 8 ml (see IEA)
CIEA, as soon as T10 level stabilized:
- Bupivacaine 0.125% + Fentanyl 0.0001% 10 to 12 ml/h
or
- Bupivacaine 0.0625% + Fentanyl 0.0002% 12.5 ± 2.5 ml/h
